Groundbreaking News: Hundreds of RX Drugs Still on the Market After Fraudulent Trials

A significant number of FDA-approved medications are based on data from questionable or outright fraudulent clinical trials. This alarming discovery stems from a recent FDA notice to drug manufacturers in June 2024, revealing that Synapse Labs, a contract research organization (CRO) in India, falsified data. Consequently, the FDA has mandated that studies conducted by Synapse Labs must be redone to confirm the safety and efficacy of the affected medications. While the FDA is being secretive about the impacted drugs, a Bloomberg investigation discovered that common generics like atorvastatin (Lipitor) and sildenafil (Viagra) were among those implicated in the fraudulent trials.

These medications are widely prescribed, increasing the urgency to verify their safety and efficacy.

While this revelation raises serious concerns about the safety and efficacy of these drugs, it is sadly not an isolated incident. 

Another recent prominent case involves the hypertension drug Valsartan. In 2018, several batches were found to contain a cancer-causing impurity, leading to widespread recalls.

"Fraudulent data in medical research is a big problem and leads us down the wrong path," warns Arch Mainous III, PhD, a professor at the University of Florida.

How Do These Drugs Make It to the Market?

The process of getting a drug approved by the FDA involves rigorous clinical trials. However, when these trials are compromised, either through manipulation of data or other fraudulent activities such as conflicts of interest on who’s actually running the study, unsafe drugs can slip through the cracks. The chemotherapy drug Docetaxel is another example, having been linked to fraudulent trial data that overstated its benefits and understated its risks.

In the case of Lipitor and Viagra generics, a Contract Research Organization (CRO) was used called Synapse Labs, as explained earlier. But for those not familiar with the term, a CRO is a company hired by pharmaceutical manufacturers to conduct studies on drugs. CROs specialize in managing clinical trials for new, brand-name drugs, ensuring they are safe and effective. For generic drugs, they perform bioequivalence studies to demonstrate that the new generic is chemically equivalent to an already approved drug.

Who Detected This Fraud?

The fraudulent activity was first uncovered in Europe when Spanish regulators flagged the issue to the European Medicines Agency (EMA) in July 2023. The EMA, similar to the FDA in the U.S., promptly launched an investigation.

By early 2024, the EMA recommended halting nearly 400 drugs until their safety and efficacy could be re-evaluated.

Soon after, the FDA began its own investigation. From February to June 2024, the FDA issued several reports to Synapse, revealing disturbing signs of fraud. They discovered nearly identical data among study participants, unusual trends in data distribution, and evidence that data had been tampered with and reanalyzed. These findings suggested significant data manipulation.

Real-Life Impact: Drugs Still on Shelves

It's not just Lipitor and Viagra generics. And it’s not just Valsartan and Docetaxel either, unfortunately. The diabetes medication Metformin has also come under scrutiny. In 2020, some versions were found to contain unacceptable levels of NDMA, a probable human carcinogen. Despite these issues, many affected drugs remain available on the market, putting patients at risk.

Why is it that these drugs are still being sold despite known issues? 

The answer lies in the complex web of regulatory and economic factors that make it challenging to swiftly remove these medications from the market.

"If the data used to approve these generics might be tainted, then the FDA should require the drugs to be withdrawn until their safety and efficacy can be confirmed," says Suzanne Robotti, founder of MedShadow and a member of the FDA Drug Safety and Risk Management Committee.

Are Medications Tested by Synapse Labs Safe?

The FDA monitors the Adverse Event Reporting System (FAERS) for unexpected side effects from approved drugs. To date, no significant safety concerns have been flagged for drugs tested by Synapse. However, the FDA has requested that manufacturers either withdraw the affected drugs or redo the studies.
